Use this form to register for the club.
Alberta Volleyball Waiver Form
This form must be completed by all EKVC athletes, parents, coaches and team
managers.

Deposit
Form
Use this form to itemize your deposits. Expenses must be
processed through the EKVC so that everything is accountable. Cheques should be
payable to the EKVC.
All Deposits
need to be dropped off with the club Accountant or Administrator. To arrange

Cheque
Request Form
Use this form to request reimbursement for expenses paid.
An associated deposit must accompany or precede the request.

club PO Box or dropped off with the club Accountant or Administrator. To arrange a drop off time and location, please email

Season Planner
This form is useful to determine who is planning to go to the tournaments, how they
plan to get there and where they will be staying so that rooms can be booked and
transportation arranged.
